What is MEDIF?
If you or the passenger whom you are purchasing an airline ticket for has a specific health condition and/or needs special assistance during the flight, you must present a Medical Information Form (MEDIF) filled out by this passenger’s doctor as stated in article 9 of ANAC Resolution no. 280 of 2013. Below are described some examples of possible health conditions that apply:
- Suffering from an illness or an impairment that causes effects on your health and well-being or even other passengers and crew;
- Having undergone surgery recently;
- Having a health condition considered unstable;
- Representing a risk to your own safety or other passengers and/or crew;
- Needing attention or medical follow-up and/or special equipment during the flight.
The MEDIF form is available for download at the end of this page, and clients must make sure that they fill it out completely and have it signed by the passenger’s doctor before issuing their ticket. This form must be sent to AZUL at least 72 working hours before the flight via our website (at the end of this page). The medical documents and MEDIF will be analyzed by AZUL’s medical professionals who specialize in aviation medicine, with a 48-hour answer deadline. It is worth remembering that the air transportation of the client described above is subject to previous authorization from the AZUL’s medical department, which will be based on the information described in the form by the signing doctor. In the event that the minimum medical safety demands are not met, passenger transport shall be refused, as stated in paragraph 1 of article 6 of the aforementioned Resolution.

MEDIF
The MEDIF (Medical Information Form) is a medical form stating that a passenger has a specific health condition and/or needs special assistance during the flight.
In-Flight Aide
Passengers with disabilities or restricted mobility due to either a mental or intellectual impairment; passengers who cannot understand the flight safety instructions; and passengers who cannot fulfill their physiological needs without assistance must be accompanied by a person responsible for them who is of age and is at full physical and clinical capacity.
MEDIF Assessment
The medical documents and MEDIF will be analyzed by AZUL’s medical professionals specialized in aviation medicine, with a 48-hour answer deadline. It is worth remembering that the air transportation of the client described above is subject to previous authorization from AZUL’s medical department, which will be based on the information described in the form.

Answer all questions correctly, with legible handwriting. Put an X in the Yes or No options. After that, all copies of the form must be signed.
The following medical conditions do not require medical authorization (MEDIF or FREMEC), as long as they are controlled and stable:
- Diabetes Mellitus;
- High blood pressure or high cholesterol;
- Arthritis;
- Artificial limbs.

Sending the form
Fill out Parts 2, 3, 4 and 5 of the MEDIF and follow the sending instructions below. Please bear in mind that the form is valid for 30 days after AZUL’s medical authorization for domestic and international flights.
Buy your ticket via Customer Service (4003-1118) and inform your MEDIF number when you book it;
At boarding, all you need to do is present your documents. Presenting a doctor’s note or an authorization is not required.
