What is FREMEC?
The FREMEC (Frequent Traveller Medical Card) is a document that makes life easier for frequent passengers who have special, permanent medical conditions, which are not serious and are stable.
Below are examples of assistance to people with reduced mobility due to a permanent, stable disability:
- Visual impairment - you must attach an ophthalmological report informing the passenger’s visual acuity carried out in the last 12 months;
- Hearing impairment - you must attach an audiometric report carried out in the last 12 months;
- Autism Spectrum Disorder (ASD) - you must attach a report from your doctor, informing the absence of comorbidities and the stable nature of presentation, carried out in the last 12 months.
After the medical assessment on the required conditions are filled out, the FREMEC card will be sent to the passenger, and will be valid for 1 year. Once a passenger has their card, they will not need to present their MEDIF (Medical Information Form) any longer; just their FREMEC, an identification document, and a plane ticket confirmation. This will ensure a speedy service and boarding.
If a passenger has an acute disease, an unstable chronic disease, or a pre- or postoperative condition, they must fill out their MEDIF, as described here.
The following medical conditions do not require medical authorization (MEDIF or FREMEC), as long as they are controlled and stable:
- Diabetes Mellitus;
- High blood pressure or high cholesterol;
Arthritis; - Artificial limbs.
It must be pointed out that the FREMEC card will not be issued for passengers with new needs (an occasion when a new assessment must be carried out), or for passengers who require oxygen supplementation for on-board use.
Important: The passengers who have a FREMEC card must inform AZUL immediately whenever their medical condition either changes or becomes more serious.
The FREMEC is available for download at the end of this page, and clients must make sure that they fill it out completely and have it signed by the passenger’s personal doctor before booking their ticket. This form must be sent to AZUL at least 7 working days before the flight via our website. The medical documents and FREMEC will be analyzed by AZUL’s medical professionals specialized in aviation medicine, within 07 working days.
It must be pointed out that a passenger who is non-eligible for FREMEC will not be prevented from boarding, but they must send in their MEDIF (keeping in mind the deadline for analysis). Please bear in mind that the FREMEC (and a medical report when necessary) must contain a signature and the passenger’s personal doctor’s CRM (this medical form will not be accepted if it is signed by psychologists, physical therapists, and other non-medical professionals without a valid CRM). If approved, the FREMEC is valid for 01 year, counting from AZUL’s answer. Only forms filled out up to 30 days before their send-in date will be valid for analysis. After 30 days, a new form must be sent for analysis. It may be issued either by Azul or another airline company, provided that the passenger’s health conditions are stable.
For assessment of a FREMEC issued by another company, the FREMEC medical files must be sent along with the authorization from the other company, where the FREMEC number/validity must be found so that this document can be verified by the AZUL’s medical team.
Passengers with disabilities or reduced mobility must be accompanied by a person responsible for them who is of age and is at full physical and clinical capacity. When health conditions are of a mental or intellectual nature, the Client requires either a MEDIF or FREMEC when such conditions compromise understanding of flight safety instructions, or when the Client cannot cater to their physiological needs without assistance. Please bear in mind that all paperwork must be analyzed by the medical team on a case by case basis.
The AZUL medical department will provide a written reply to requests for an aide made by the passenger within a 7-working day deadline. If a passenger’s FREMEC is approved, their aide shall get an 80%-discount on their fee. This discount does not apply to fees due to airport operators and/or governmental entities, as well as additional services offered by AZUL, except for those described in items I and II of paragraph 3 of article 8 of the aforementioned Resolution.
In addition, AZUL states that the data requested in the medical form are due to a legal requirement, and they are essential for making air transportation possible. Thus, the FREMEC data is treated as sensitive, and is analyzed by specialized medical professionals only. They are protected according to medical confidentiality prerogatives and passengers’ privacy.

Fill out Parts 2, 3, 4 and 5 of the FREMEC and follow the sending instructions below. Please bear in mind that the form is valid for 30 days after AZUL’s medical authorization for domestic and international flights.
Buy your ticket via Customer Service (4003-1118) and inform your FREMEC number when you book it;
At boarding, all you need to do is present your documents. A doctor’s note or an authorization is not required.
